My fields and entries are not being mapped to salesforce even though Jotform says the integration is up and running.

What is the form in question? Please try removing the integrate, then clear your forms cache: https://www.jotform.com/help/229-How-to-Clear-Your-Form-Cache
Finally, integrate back again, and submit your form to test it. If the information is still not being sent to your Salesforce account, please share a screenshot of your Salesforce configuration, example:
