A form I created has disappeared. How can I retrieve it?

When I click on the jotform link, I get a message saying that it has been deleted by the owner. I have no memory of deleting this form and cannot find it in the trash or archive. Form has "accountability" in the title. Please help! Thank you.

That form was deleted, but you can find it in Trash folder:

And restore it from there:
