How can my users fill out my form?

Is there an app where a person can fill out a form?  I am creating a form that would be filled whenever our partners have a request for materials so I wanted something they can click on there phone and it would go directly to the form.  Thanks.

Your customers will use a browser to fill out your forms. All forms you create on JotForm will have its own unique link. Users who click on that link (your FORM URL) will be able to submit the form for as long as they have a browser and an internet connection.

You won't need an app per se - JotForm is an online form builder so your users/customers will usually fill out forms on a web browser.
