I used your program to create a form for a client, fraserlandscapecontracting.com found at this URL http://fraserlandscapecontracting.com/quote-request/
I created an email info@fraserlandscapecontracting.com and when I tested the form the response did not show up in their email box.
Also when I tried to submit the form a second time, I got a message that said
Sorry! Only one entry is allowed. Multiple submissions are disabled for this form.What do I need to do to have this form work and the results show up in the clients email box

I checked your email address in our bounce list and it is not listed. Have you tried checking the spam/junk folder ?
Checked your mail logs, and I can't seem to find any emails sent.
Kindly disable your Unique Submission,to fix the issue of not to be able to submit the form again.
