How to setup form with options to select quantity?

    I want to use a supply request form internally. It needs to allow them to choose the items they need and the quantity. The information in this form needs to come to me so I can then order what is needed. How can I set up a form to allow this? There is no payment involved for this.

    I have set up a form just with multiple options to choose, but I realized there is no way to pick a quantity for each item they choose.

    Would this work for each item in the list? I have grouped supplies together and they can select more than one item, but I need a way for them to tell me how many of each item they need. Is that possible? If so, how do I set it up?

    This should work to collect each item per item. The user can select one color and define the quantity then select add to add another option to select another color with quantity.

    You can use single choice,so that it won't confuse your user.
