Hello! We have our adoption questionnaire on our website using Jot Form and we have not been receiving completed forms when people submit them through their mobile devices. Is this something you can help us with?

I was able to replicate the issue on my Android phone.
It looks like your website is not responsive and the form is a bit hard to fill on mobile phones.
I suggest you redirect the users to the standalone Form URL which is responsive and your users shouldn't have any problem filling the form there. http://www.jotform.us/form/31270168047147
If you still want to keep the form embedded on your website, can you please tell us which embed method did you use? Can you try using one of these methods?
