I see that one of the auto-renew action had failed. Here is the reason: Cardholder transaction not permitted.
On the other hand, it seems the problem has been fixed and your subscription renewed today. There were two active subscriptions in your account and one of them has been cancelled and made a refund for it.
Your JotForm account is in a good standing now. The next billing date is on 24th April 2018. Please let us know if you have further questions.
