I have cloned your form to my account for further testing. I realized that you are using the Quiz Form App for your form. I re-set my clone forms connection with the app. I was able to receive all e-mails (notification e-mail and Quiz Form app's e-mail) correctly.
- Could you check your spam/junk folders?
- You may need to re-set your Quiz Form App as you can see in the below screencast:
