Account: Autosuspended - Please reactivate

    I am trying to get into my JotForm account. I cannot get it and when I try to login it will say login failed - autosuspended.

    I am hoping to rectify this as I am using JotForm for a project this term at school.

    Your account was suspended because your form: Northeast Regional Ambulance - IT Ticket - 80657213040144 was found to be collecting username/passwords. 

    According to our terms of use this is forbidden.

    Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Collecting Sensitive Information. You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials.

    In order to proceed we must ask for your consent to delete the password field from your form.

    Once we remove the offending field we can reactivate your account.
