I am trying to run a test of my new form:I have read several forums of this issue happening. I have cleared my browser cookies and cache and tried multiple browsers, but am still experiencing the problem. I do not need to embed this form anywhere. It will be distributed as the above URL.Page URL:

I have checked your form and the issue was with the Appointment Slots widget.
The form should be working correctly now. I have made a test submission to your form which you can delete.
