I'm trying to find a way to make the fields listed below my checkboxes only appear when the checkbox is checked. I tried to use conditional formatting but it didn't work. The boxes disappear and don't show up when the checkbox is clicked. What am I doing wrong? Here is my sample form: http://form.jotformpro.com/form/22995424304960

Hello, I've taken a look at your form and I can currently see what your problem is. Could you please specify which fields you are trying to hide based on certain conditions when a checkbox is ticked this way we can help you to create the form you are trying to make?
