I was suspended, I believe because I used the word password in one of the questions, but it was not for degenerative purposes, just a way to identify and ensure who was sent the form

Our phishing detection system detects the problematic fields on your forms. You need to allow us to delete your guilty field from your form in order to re-activate your account.

You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."
