    Hi team

    Please can you help to check and advise the reasons of suspending our accounts? We also would like to upgrade to Bronze.

    Your account was suspended its because you are collecting Passport IDs. It violets our Terms of Use.

    "Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

    You need to allow us to delete your guilty field from your form in order to activate your account. However, if you really need to collect this information from the user, then you need to add the following to the form:

    You need to provide an official proof us why you need this field on your form. You can send your proof documents to support@jotform.com. It would be great to follow if you can mention this thread's link in your e-mail.

    1. You need to add a Terms widget to your form. You can use the Short Scrollable Terms widget or Terms & Conditions widget.

    2. You need to add a Digital signature widget to your form. You can use the E-Signature widget or the Smooth Signature widget.

    Your proof will be evaluated, and if it is accepted, you can collect this information by adding mentioned widgets to your form.

    Thank you. Please remove the section that asking for the passport in our form and re-activate our account.

    Thank you for your feedback. I have deleted fields related to Passport/ID as seen below.
