How to save as a pdf form without losing formatting?

    I create my form and it looks great with my logo and with the formatting. When I use the "save as PDF form" it seems to lose the formatting and the logo.  I can still use the form and save it as pdf...but it looks awful.  


    How can I save it and not lose the formatting. I've included a screen shot of the pdf file (that looks so bad) and here's the link to the form itself.

    If you only need a PDF copy of the form and not a fillable copy that can be submitted, add a print button to the form and then use the Print to PDF function:

    That will give a PDF of the form that looks exactly like the form.
