I was wanting to put my membership on hold for about 3 months - or downgrade to the free version as I won't be using it for the next few months but will need to kick my membership back off in a few months time.
If I downgrade my membership to the free version will I lose the forms that I have created in the Bronze membership?

No, you will not lose the forms. On regards of the already collected submissions, a free account allows storing up to 500 submissions, which is enough in your case, since your total submissions is 12 so far.
So, please confirm if you would like us to cancel your subscription, put your account back to free, and refund you the recent charge.

So when I decide to upgrade again in a few months time, will the form I have created still be available for use - meaning the blank form that I have created and the link for people to complete it - will it still be there and saved to my account and okay for me to send out to have people complete it or will I need to recreate it again?

Your forms and submissions will remain on your account forever (Paid or Free account) unless you remove it.
I checked your account and you're still subscribed to the Bronze plan. Let us know if you want us to downgrade your account. Also, you can upgrade anytime by following this guide: https://www.jotform.com/help/331-How-to-Upgrade-My-Account

I downgraded your account and give a refund for it. Please allow around 4-6 business days for you to see the charge reflected back to your credit card.
