Created a form for story submissions on my website about 2 months ago. Tested it and it was working fine. Tonight, I attempted to log in to my JotForm account and first received a "suspended account" error. I was able to log in after resetting the password but I noticed the form I created was no longer listed. I proceeded to my website (link below) to check the form/functionality (Submit Your Story) and received a 404 error. I currently have a Bronze account. Please advise.

It seems that you have 2 accounts. The account BereaBPW witch has a Bronze subscription and a Free account Daneene.
Copyright Infringement. Company respects the intellectual property rights of others. Accordingly, Company has a policy of disabling access to any Submission that violates copyright law, suspending access to the JotForm Service to any user who uses the JotForm Service in violation of copyright law, and/or terminating in appropriate circumstances the account of any user who uses the JotForm Service in violation of copyright law.
You have to agree to our terms and only then your account can be activated. The logo will also have to be deleted from the forms.
If you have a proof that you have the rights use that logo, please let us know and provide us a proof.

We have now removed the logos from the forms and activated your account. You should be now able to login to your account. If you need to use the logos on the form, it is required to change your account email address with the respective business email address.
