I tested your 'NEW LEAD' form as you can see in the below. I was able to see the changes in preview mode.
Are you still experiencing the problem? Which action are you not able to do?

Unfortunately, changing the Address field to a Dropdown box is not possible. However, you can use the Dropdown field and change the Label to Address. Here's a screen animation guide:

Unfortunately, it is not possible to customize the fields to be synced with Google spreadsheet. The Google spreadsheet integration sends all the form fields to the integrated spreadsheet automatically. Please do not make any modifications to the integrated spreadsheet which would break the integration. Instead, you may consider copying the spreadsheet data to a new sheet and make modifications like deleting or hiding the columns.
