I would suggest you to contact the person that created the account and ask for login credentials.
If you provide us a link of the form, we can tell you in which account it was created.

The form you provide was created in the account amfrazier. The email address of the account is ap********on@at***ta.k****a.us
I saw that the account email address and password were changed yesterday.
You can log in to the account with the email address or with the username. If you don't know the password, but you have an access to the account email address, you can reset the password.
If you do not have an access to that email address, I would suggest you to contact your IT department and see if they can provide you an access to that email address.
You can also contact a person that created that account and ask for account login credentials.
