Form is not submitting

Please check our form Client Tax Profile /80376662001147 it's not allowing our client to submit the form.  It also has problems when using internet explorer.

May we know if your form is embedded to your website? If yes, can we have the website URL so that we can check it out?

Please ask your client if he is using any web security or popup blocker, it  may be the cause of the form not submitting. You can suggest, whitelisting submit.jotform.com in his web security.
