Need Configurable List laid out

Hi. I have emailed 2 or 3 times asking for assistance with this and have yet to receive a response. 

If you type in 5501 where it says TECH you will be able to see what I am talking about.

Under Findings I need this to look better.

I would like everything to be on one page instead of having to scroll over to the right, so you can bring it down to more lines

I need the lines to be straight and not diagonal.It looks like only the 2nd line is diagonal but the 1st and 3rd are off the page causing me to have to scroll over, so maybe there should be 4 or 5 lines to fix this issue. It is OKAY on my mobile device but looks terrible on my computer, which is where I do most of my editing. 

Where it says ON DEPARTURE that should basically just be a headline, or look like one. I do not need a yes/no or a text box. I just need it to say ON DEPARTURE with everything else under it. 

Also where it says findings I am unable to delete the 1 next to the word FINDINGS. 

Please help me with this asap it is very important. Thank you :)
