What is the best form format for an Audit/Test

Good afternoon,

We are wanting to utilize Jotform to conduct a multiple choice test of competence and overall fit for our employees as a way to grade them and have usable data through the utilization of a multiple choice format. 

How would we best achieve this using your systems?
