Why Square integration fields are not working properly?

Hello! 

I have a form all set and it is connected to square.  When I publish the form, the form will let me enter my name for the payment, but it won't let me enter any other card information like the number, expiration, etc.  

Can you please advise?

The issue is with disabled products that you have:

So just remove those fields and it should be working fine.
