People submitting are having trouble with uploading images.

    I can not find that upload field on the form you provided. 

    The form you provide only has "Product Image" file upload field.

    As you can see I successfully uploaded my image. I also successfully submitted the form.

    That form was also not created in your account. It was created in the account that is sharing the forms with your account.

    I checked the forms in your account and I think that the screenshot you provided if from this form - http://www.jotform.us/form/73403716084151 

    I tested that form but I was still unable to replicate the issue you reported.
