It seems my account has been suspended for some reason. I can't access it anymore even after resetting my password.

I notice that the account is suspended due to the form http://www.jotform.co/form/80266747784874 which is collecting Photo ID which is not allowed with JotForm. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our terms of usage for more information on phishing.
It is required to remove the guilty fields on the form in order to activate your account. Please confirm so that we can remove the guilty fields and activate your account.

Yes, sure you can remove the Photo ID field.
