Why my account has been suspended?

    I signed up yesterday and verified my account, now when i try to log in, it says autosuspended?

    Your account was autosuspended because of your "Alternative Medicine-Patient Intake" form and the reason is that you were collecting Social Security Numbers which is considered as sensitive information.

    Could you please let us know the purpose of collecting such data so we can review the possibility of re-activating your account.

    The form was one of your templates that I had just chosen and was just getting ready to edit. The social security option was already on the form that was suggested by your site.

    Sorry for the inconvenience. 

    The form template you choose is probably not updated. We just recently need further information and details why the form needs to collect SSN. 

    I have removed the SSN field to your form and re-activated your account back to good status. You can now use your account and manage your forms properly. If you want to collect SSN, please let us know so we can provide you further information to avoid getting suspended again.
