I need the Square form to have my clienst pay for their yacht rentals

I HAVE A SQUARE ACCOUNT FOIR MY HAMPTONSBOATRENTAL BUSINESS AND I NEED A FORM TO ACCEPT PAYMENTS TO MY SQUARE .    CAN YOU SHARE A FORM?   HOW do i use square on this
