I am not able to login to my account

    I am not able to log into my account since yesterday. At first, my IP address was wrongfully banned for phishing attempts  -->  https://prnt.sc/izc4dr 

    Then finally the ban was released. But when I try to login with my username and password, it would not let me in - your system would not accept my old password.  

    I requested a password reset. I changed my old password, and I am still not able to sign in.  -->  https://prnt.sc/j00png

    Please provide assistance to log into my profile. I have only ONE form created which I spent a few hours to build. I need this form badly.

    Plese, advise.

    I see that your account is suspended due to the form http://www.jotform.us/form/80166134607150 which is collecting Credit card information on plain text box fields. Unfortunately, it is not allowed to collect credit card information on plain text fields using JotForm. Such forms are considered as phishing forms and associated accounts will be automatically suspended. Please refer to our terms of usage for more information on phishing. If you need to receive payments, it is required to use one of the payment methods available in the form builder. These payment methods comply with the PCI/DSS security standards to process the payments securely.

    We have now deleted the guilty fields on the form including SSN number fields and activated your account. You may now login to your account without any issue. Please refrain from adding such fields to your forms to avoid any further suspension.
