Form Disabled

    Can you please help me as my form is disabled. Is this due to a payment needing to be made?

    I have checked your account and it seems it has been reported by our phishing detection system as suspicious of phishing activities, your from "Tofoya & Associates Invoices" is the one that triggered the suspension.

    It has fields with banking information and that violates our Terms-of-Use.

    We understand this might not be the case with your form,  yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

    In order to reactivate your account, we will have to delete those guilty fields.
    And if you want to add those fields again, we encourage you to follow this recommendation:

    Add a sort agreement and/or an e-signature field to the form, so your customers will agree to provide their Banking information along with personal info.

    Hi Admin,


    There is a signature field at the bottom of the form. I have deleted the fields however still cannot access the account.
