My login is suspended

Afternoon,

The email j.reimers@vertex.capital is now an old email (but still linked to our JotForm account) however all our important client application forms are on that login. Is there anyway I can still access them? This is essential to the operation of our business and took days to compile and complete these online forms from our paper forms.

I see that your account vertexcapital registered using the email address j.reimers@vertex.capital is suspended due to the form http://www.jotformeu.com/form/72955130943358. This form is having the upload fields collecting sensitive information which is not allowed. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.  

In order to activate your account, it is required to delete the guilty fields from the form. Please confirm so that we can delete the guilty fields and activate your account.
