Account suspension

    Hi! I recently created a restaurant application form on Jotform. Usually when I went to Jotform I didn't have to log in, but last time it made me, and I had forgotten both my username and password. I tried a few different things, and then it ended up locking me out of the account. I need to get back on because the restaurant is opening soon, and I need to see the applications.

    I see that the account is suspended due to the form http://www.jotform.us/form/80635608590158 which is collecting SSN number on the form. It is not allowed to create login forms or collect sensitive information like Credit card numbers, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.   


    In order to activate your account, it is required to delete the SSN number field from the form. Please confirm so that we can delete the field and activate your account.

    Hello both of my forms are suspended and I asked almost 2 hours to find out why, can you please shed some light on why both of my forms are disabled and my account is suspended? I have had the account for almost a year without issue. No email notification of why it was suspended or even when.
