Portions of my jotform have disappeared

2) Portions of my jotform have disappeared - namely the section where donors decide which boxes of cards they would like to get for the fundraiser. Because it was an imported event, I cannot figure out how or where to add them back in.

I checked the revision history of that form and you changed Payment Type your Square payment field from Sell Products to Collect Donations. Because of that, your products were removed from the form. 

You can revert your form to the version before you made this change to resolve this issue.
