Cloned your form but I can't seem to replicate the issue.
I can edit the form in the form builder.
Cleared your form cache.
Let us know if the issue still remains.

I can't edit in Google Chrome (get blank screen) but I have been able to edit it in Microsoft Edge.

Microsoft Edge so that's what I've done.

I just checked your form in edit mode using Chrome browser, and it loaded fine. Please try clearing your Chrome's cache: https://support.google.com/accounts/answer/32050?co=GENIE.Platform%3DDesktop&hl=en
