Why my autoresponder is not working?

I just setup 1 form with an email notification that works fine. But the autoresponder email is not being sent out. If I do the "test" email, it is being sent. But if I fill in the form, only the notification email is sent. What have I done wrong in the settings. My username is: albertsa

I checked your form and the issue is that your Autoresponder is set to be sent to Full name field:

And it should be set to be sent to Email field:

I changed that for you, so your Autoresponder should work fine now.
