Why isn't my form displaying on my web page?

I have been testing your page and I can see the form appears when loading the page, but when  it finishes the form disappears: 

In order to make sure it's not a conflict with the JotForm code, I would recommend you to try adding a different element such as a text, image or another element that can be displayed on that form's section (note that you will need to remove the forms code).
