I want to make a field on the new card forms which shows an image (either icon or image) with an input or dropdown for quantity.
so show three photos for examples of small , medium and large (rather than spelling the words) and allow multiple choices with various values. ie: 2 small and 1 large.
so kind of like an input table with image choices.
Any help would be appreciated.

That can be achieved with the payment field.
You can add an image to your product and allow your users to select quantity, using a text box or dropdown field.
If you don't want to process payments, you can use Purchase Order payment tool:

Thank you but I don't want to process payments based on these items and I can't add the Purchase order widget because I already have Stripe to process payment. My payments are calculated by distance.
The images are to show the difference between small, medium and large packages and then a dropdown to show how many of each are being delivered.
Is there a way to add images to a multi line question? or an input table?
any other suggestions.

I'm afraid both the CARDS and CLASSIC layouts don't have this option. With the Classic Layout though, you can potentially use the TEXT FIELD or the IMAGE FIELD to display the images, then strategically place them side by side, through field positioning, with the input field you intend to use (e.g. a dropdown field).

If you want, we can put this up as a feature request. We already have an IMAGE CHOICE field for Card Forms that I noticed you fiddled around with on the last card on this form. But perhaps adding an image field to the MULTILINE QUESTION would be a better addition.
Let us know your thoughts on which one you prefer and why and we'll take it from there.

Thank you for your efforts. I personally think that the more flexibility one has with multiline question the more options and creations they could come up with. However for my personal issue an image choice with individual dropdown for an amount would be best and I could see this being valuable for lots of users.
