Is $19 subscription a once of fee or monthly?

The $19 subscription is it a once of free or monthly? 

As we just need it for 1 month until further notice  

Please be noted that JotForm is a subscription based service and will be renewed automatically as per the plan chosen. If you need the subscription plan for only one month, you'll be required to cancel the subscription manually after upgrading the account and before renewal.
