Why is our cloned form in different layout to original form?

HI,

I have cloned a form but the styling has disappeared?

Please see the below image, only the text/structure remains.

I have tried clearing all history/cache but this hasn't improved the situation.

Any help would be greatly appreciated.

I have seen the form and seems you were cloning form in card layout instead your form is in Legacy Layout. Kindly note we have forms in two different layouts and you must clone your form to the exact layout of the original form.

I would recommend you either change your form to card layout as shown below:

If the above does not resolve the issue, I would recommend you clone the form afresh as shown below.
