My website (http://ultrex.net/support/) uses a few forms to allow customers to submit information to us. The embed code loads up when they select the type of form they need. Problem is, only Mozilla loads the script- not chrome, edge, or safari. This has happened a few times randomly already and has seemed to fix itself a day or two later. The webpage was built using frontpage. Any ideas?

I checked on your website and I was able to see the problem as well.
The multiple embedded forms on your website only load if using Firefox browser but not on other browsers like Chrome, Safari or IE11.
On further test I was able to identify the caused of issue. It was due to the multiple embedded form on the same website page.
Because the forms were using JS script embed code, the script libraries were conflicting and thus prevents the script functions working properly.
To fix, I recommend you embed each of the form on a separate page and iframe.
You can also use the iframe embed code of each form.
