Most widgets are not working on my account

I am unable to use widgets previously working (any list, configurable list, etc). When i try to add new widgets to forms they do not work. Dialog appears, the options are available, but the widget does not show on the form. 

I created a test form and attempted to add various widgets. None of them work. Form is below. 

The widgets appear to be loading properly at the moment:
