My account has been suspended and I’ve been accused of phishing

    My account has been suspended and I’ve been accused of phishing. I don’t even know what that is or how to do it. The most recent form I was using did link my Sqaure account to take donations for a fundraiser. But Ive done this in the past with no issues. I received no warning and can’t access any parts of my account. This is unacceptable. Please help!! 

    Attached is a photo of what happens when I try to log in from my computer. But I can’t even complete the form because it blocks my ability to see the Captcha image

    Your account got suspended because of the "Usborne Order Form - Meredith Carter" form is collecting credit card information using Basic fields. As per our terms of use, collecting credit card information is not allowed here in JotForm.

    I have activated your account back to good status. Please Purge the guilty form in your Trash to avoid getting suspended again.

    Meredith Carter is not my name and that is not my account. I see that my account has been reinstated, which I appreciate. I think potentially Meredith copied my form and then changed questions. I reached out to her to tell her to delete that form. Thank you.

    Just to clarify the situation. Meredith Carter is not the username, it is a part of your form's name. The form is called "Usborne Order Form - Meredith Carter" and is located at the Trash folder of your account. Even if you have created or cloned that form by mistake, you need to purge it anyway. Otherwise, your account will be autosuspended by our phishing detector again.

    Please check your Trash folder to see a form there.

    Thank you so much for clarifying! I had no idea those were even in my trash. It is purged now. Thank you again!
