Getting Only one entry is allowed error when submitting form

I submitted a ticket yesterday about the signatures not working on my form via phones.

Now, when I or others test the form on the webiste we get the following message:

warning Screenshot 10Sorry! Only one entry is allowed.

I'm not sure if I'm doing something wrong, but I REALLY need this form to work quickly as I'm using it for time-sensitive enrollment purposes.

Here's the link to the form:

No matter what we select, we always end up with the same error message. Also, the phone signature issue is not fixed.

I hope you are able to help me quickly!

    Best,

    Julie Schroer

    Julie,

The reason for the "Only one entry allowed" submission alert from the form was due to the Unique Submissions feature being enabled on your form https://www.jotform.us/form/80936330313147

You can disable the feature if you want to allow multiple non unique submissions.

I hope this help. Let us know if you need further assistance.
