How can I delete the last option on the multiple choice field?

    If you go to the last step on this form, you will see a checkbox called "checkbox"


    why would this be there and how do I get rid of it?

    Go to your FORM BUILDER > click the last field > click the GEAR icon > go to the OPTIONS tab > then delete the last line (checkbox).
