I can see there are two accounts related to your email address, the form on the link you shared belongs to your free account.
In order to allow access to the form you may proceed like this:
1. Reset the password using the account username: abqhomesearch
https://www.jotform.com/answers/290640-Password-Protect-form
2. We may move all the forms under your free account to the paid one and delete the free one.
