Cannot find my username and the forms are showing not found

    I cannot find my user name and the forms are coming up, not found. What is going on?

    Your account (Echevarriatravel) is currently suspended. This form (Travel Disclaimer and Insurance Waiver) triggered the suspension.

    Please take note that collecting credit card details using regular text fields are not allowed here in JotForm. I recommend you to check our terms of use.

    We need to delete the guilty fields so that we can reactivate your account. If that is okay with you, please let us know.

    If you need to collect payments from your users, please use one of our payment processors. Here's a guide: Setting-Up-Your-First-Order-Form. You may also capture the card details and charge the user at a later date. Here's a guide: How-to-Enable-Payment-Authorization.

    Okay, so what do I do know. I will create it without credit cards. I was using another travel agents recommend form.

    The username for the account was listed in my colleagues original response.  The username is "Echevarriatravel" without the quotes.
