"Your account was reported by our phishing detection system as suspicious of phishing activities, your form "Credit Card Authorization Form" is the one that triggered the suspension.
It has fields that are asking for Credit card information, and under our Terms-of-Use this is prohibited.
To lift the suspension, we need to remove the guilty fields.
