Display issue on email Forms

we create a template form for my gym and then we copied it and use it for other forms, that we needed, we have this little bug, or config issue which popup all the email all the headers have my Gym attached pls see attached screen shot 

Dont worry :) I found it the copy has kept the variable value instead of the variable {activity} its all working now
