I have set up a condition to auto respond once a certain box is ticked however when testing we are not getting the auto response sent to the email address specified earlier on in the form, is there anything I am doing wrong?

I cloned your form to test it and I was not able to replicate the issue. Here's a screencast of my test and the email result:
Please take note that you need to edit the form and select the "YES" option in the "Instrument is now ready to collect" field.
I also submitted a test submission to your form, please try editing it and follow the instruction above to trigger the email condition.
