Unable to login. Account got suspended.

    I work at YWAM Nashville and use jotform to create reference forms for school applicants. The other day I attempted to send a reference form and then a prompt came up saying that my ip address had been banned because of a phishing attempt. We need this lifted so we can carry out our reference check system.

    The reason that your form was disabled and your account got suspended was due to the passport number field on the form. We do not allow for the collection of sensitive personal information such as credit card details, social security numbers (SSN), or passport IDs.

    You can collect passenger names, addresses, emails, phone numbers, their citizenship, and other information that is not about their SSN / Credit Cards (without any payment tools) / personal ID.

    To reactivate your account, we need to delete this guilty field. Please confirm that you agree to our Terms of Use and let us delete these guilty fields.
