You can configure this using our Short Text Entry field then calculation.

Can you also clarify the items? I tried creating a demo form for you, but I do not understand the options. Are these correct?

Number of Nights ($25/night)?

Extra Adults ($10/adult)?

GST? What is the rate? 

 I have finished creating a form  JotForm    Can I make the whole page visible and usable in my website?

I have checked your JotForm and see the form is currently using Classic form layout which displays all the fields in one page. You may embed the form on your web page so that the form should be displaying in a single page.
