Google spreadsheet integration lost

I have just discovered that the google sheets integration I had with one of my forms has disconnected. I can't seem to figure out how to re-connect it without it creating a new spreadsheet in google sheets. I really need it to reconnect to the original sheet. Can you help?

I can see that you have two forms in your account but none is integrated with Google Spreadsheets.

Unfortunately, if the integration was removed, it is not possible to reintegrate the form with the same spreadsheet. A new one will be created.

The integration can break if you have made a change in your form (such as adding/removing/changing a question) or directly in your spreadsheet.

If you want to make edits to the spreadsheet, please use the IMPORTRANGE function in another form and make the changes there.
